Learning objectives
- Achieve autonomy in the multi-professional management of the frail patient both in the diagnostic phase and in monitoring over time
- Recognize and intercept the determinants of frailty early.
- Develop the correct relational and interview patterns with the patient and his caregivers

Course unit content
The course will analyze the concept of "frailty", the diagnostic criteria and the main typologies (e.g. physical, cognitive, social frailty, etc.). It will provide the main diagnostic and interpretation tools. It will provide multidisciplinary and intersectoral operational insights.
Part of the course will focus on communication aspects towards both the frail patient and his caregivers

- The evolution of the patient (and of the social-health system)
- The concept of frailty in medicine. Definitions, diagnostic criteria, diagnostic tools
- The relationship between frailty and illness. Geriatric syndromes. Notes on physiopathology.
- Social care needs and settings of the fragile patient. Home, nursing home, Social-rehabilitation centres, Family homes, Apartment/co-housing groups, Hospital
- Professional expectations, patient expectations and caregiver expectations
- Empathic communication, diagnostic and therapeutic role. Communication techniques. The risk of burnout
